Job Summary
Patient Care Coordinator Role:
- Assesses the needs of the hospice patient and family, plans, and implements nursing care for assigned patients in accordance with physician orders and the interdisciplinary plan of care.
- Coordinates end-of-life healthcare with a focus on providing pain and symptom management to enhance the quality of life.
- Addresses patient and family concerns with education, counseling and emotional support.
- Works with the interdisciplinary team to ensure timely communication of important information and care plan changes.
Admit/Flex Role
- Rounds at the hospital and facilities to present information about Hospice Maui’s philosophy and services for patients and families.
- Coordinates admissions and smooth transition from hospital, facility or home to Hospice Maui service.
- Performs physical assessment, symptom management, patient and family education, ordering of DME and medications as needed and mentoring patient/family/care givers through the dying process.
- Participates in inter-disciplinary group meetings.
Qualifications:
- Maui resident.
- Graduate of an accredited school of nursing. BSN preferred.
- Current license as a Registered Nurse in Hawaii and CPR certification.
- Two (2) years of experience as an RN preferred to include hospice, home health, medical, surgical or critical care. (Preferred)
- Experience with pain and symptom management to include infusion pumps.
- Licensed driver with a clean driving record and an automobile that is in good working order and insured in accordance with state requirements.
- Possess excellent observation, critical thinking, time management and communication skills along with cultural sensitivity and a deep understanding of the dying process.
- Candidates must be able to both effectively collaborate with the interdisciplinary team and work independently without supervision.
- Technical skills with electronic medical records systems, smart phones and iPads for documentation and communication.
- Prolonged or considerable walking or standing. Able to lift, position and transfer patients. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
