Job Title: Patient Care Coordinator

Job Summary:
The Patient Care Coordinator (PCC) assists patients and their families in accessing care and services from participating physicians within Oncology Physicians Network (OPN) to ensure the best patient experience. The position requires excellent communications skills (verbal and written) and have experience in coordinating care for patients. The position will interact with various stakeholders including, physicians and their office staff, medical group clients, patients and OPN’s network operations team.
Essential Duties and Responsibilities:
- Maintains compliance with all company policies and procedures
- Answers phone calls and emails from patients, their families, and physician offices
- Makes outbound calls to patients, their families and physician offices to coordinate care
- Collaborate with medical group clients, physician offices and departments within OPN to ensure timely access to care
- Monitor and maintain reports related to patient care and grievances
- Assist in gathering information from physician offices and medical group clients as it relates to patient care coordination
- Respects patients by recognizing their rights, maintaining confidentiality
- Provides the highest level of customer service to ensure the best patient experience
- Maintain good company loyalty, set a good example, and project a healthy positive attitude
- Perform other duties as assigned
Education and/or Work Experience Requirements:
- Excellent computer proficiency (MS Office – Word, Excel, and Outlook)
- Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service
- Ability to work independently and to carry out assignments to completion within the parameters of instructions given, prescribed routines and standard accepted practices
- Bachelor’s Degree in healthcare or related field of study, required.
- 5 years of experience in customer service or healthcare, preferred.
- Oncology experience – preferred
- Excellent verbal and written communications skills, including ability to effectively communicate with internal and external customers

Physical Requirements:
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.

Job Type: Full-time
Pay: $22.00 per hour
Expected hours: 40 per week
