The Behavioral Health Therapist at Access Carroll is an integral member of the integrated health care team and supports the organization’s mission to champion and provide quality, integrated health care services for low-income residents of Carroll County, Maryland within a person-centered model and community-based population health plan. The Behavioral Health Therapist is responsible for supporting and providing direct behavioral health services under the direction of the Behavioral Health Services Medical Director and Behavioral Health Services Manager, including comprehensive assessments, diagnoses, preparation and implementation of a treatment plan.
POSITION DESCRIPTION
Essential Functions include:
· Function as a member of an integrated health care team within licensure to provide high quality and efficient behavioral health care, including substance use disorder, mental health, and case management services.
· Provide direct individual, group, and/or family therapy in accordance with accreditation standards of The Joint Commission and Behavioral Health Administration guidelines, COMAR, and ASAM.
· Conduct Behavioral Health Assessments, including screenings with appropriate documentation, resource navigation, referrals, and integrated team communication.
· Performs initial evaluation of patient using biopsychosocial treatment planning and completes Electronic Health Record upon admission and discharge. Decisions and recommendations for the Behavioral Health Therapist include:
o Decisions regarding treatment plan design in collaboration with the client.
o Recommendations regarding a more restrictive level of care for their clients are made to their immediate supervisor.
o Recommendations for referrals and medication evaluations to providers.
o Recommendations on individuals assessed to enter treatment services.
· Work collaboratively with the patient to design a treatment plan that addresses the individual’s behavioral health problems with goals including measurable criteria for completion.
· Provide individualized evidence-based treatment to a wide range of diagnoses and culturally diverse populations.
· Provide psychosocial support for immediate crises and develop collaborative care plans.
· Maintain accurate and timely progress notes per current policies within the electronic health record and other required documentation platforms, including all client contacts and client related correspondence in accordance with policies and procedures.
· Collaborate interdependently within an integrated health care team including medical, dental, mental health, and substance abuse recovery services in a diverse person-centered community clinic environment.
· Provide both program and case consultations to a variety of health, welfare and social service organizations, daily or as needed.
· Pro-actively manages caseload within a busy integrated setting that utilizes both warm and hot hand offs.
· Partner and maintain collaborative relationships with other providers and agencies within the community to provide integrated person centered care.
o External agencies and partners include: Parole and Probation, Residential facilities, Civil/District Court Personnel, Attorneys, Department of Social Services, and the Carroll County Drug Treatment Court Team.
o Collaborative relationships within the Carroll County Detention Center include: Detention Center Personnel and Chaplain, Carroll County Health Department, Corrective Facilities Medical Services, and Public Defenders.
· Participates in special assignments and committees both internal and external with supervisory approval. Reports content to senior level staff. Reports to clinical staff information as deemed appropriate by supervisors.
· Completes all necessary training, certifications and competencies as required for social work licensure and employment in the environment of care.
· Utilize equipment, machinery, and tools to complete required duties include computers, printers, telephones, fax machines, photocopiers, televisions/DVDs, and vehicles.
· Participate in the system development and reporting of safety and quality assurance measures and activities.
· Adheres to the State of Maryland Behavioral Health Care Standards within level of licensure.
· Supports and promotes a safe environment of care, including required infection control and behavioral management procedures.
· Attend trainings, staff meetings, and other organizational events.
· Other duties may be assigned as related to the Behavioral Health Therapist
· Participate in needs assessments to identify barriers to obtaining health care services, available resources, and ultimately diversion of over utilization of emergency department services.
· Provide clinical and procedural support training to other members of the health care team as related to behavioral health services.
· Participate in the system development and reporting of quality assurance measures and activities.
· Maintains and ensures reporting requirements are delivered on time.
· Demonstrates openness, team attitude, professional communication, and acceptance of change to ensure high quality patient care in an integrated care environment.
· Ensures compliance with all HIPAA requirements.
· Participate in team building activities, continuing education, and community events that support the mission of Access Carroll, Inc.
