Position: Engagement Specialist - Care & Population Health Management Location: Alexandria, Minnesota (Hybrid or In-Office)

Responsibilities:
- Ensure compliance with Minnesota Department of Human Services (DHS), Centers for Medicare & Medicaid Services (CMS), and National Committee for Quality Assurance (NCQA) requirements.
- Assist in creating a symphony of paperwork—making copies, faxing documents, and crafting letters.
- Dive into the Medicaid Management Information System (MMIS) like a data detective. Enter care coordination and Focused Wellness activities with precision.
- Reach out to our members, serenading them with the benefits of program enrollment. Connect them with our talented team of Community Health Workers, care coordinators, and specialists.
- Attend case management events, trainings, and webinars. Your mission? Enroll members into PrimeWest Health’s case management, care coordination, and Focused Wellness programs.
- Dial up members making calls of outreach and for surveys.
- Data entry in our Care Management documentation system.

C. Educational Requirements and Required Qualifications
- A minimum of 2 years customer service and/or direct patient care experience
- Health plan experience preferred
- Demonstrated knowledge of, and proficiency in, Word, Excel, and Outlook required. Experience with data entry and taking meeting minutes a plus.
- Flexibility and ability to handle multiple tasks. Demonstrated organizational skills and attention to detail. Ability to work effectively with a variety of people both within and outside of the organization. Good verbal and written communication skills.
- Ability to sit at a desk or computer station most of the time
- Ability to occasionally lift and/or carry such articles as file folders and small boxes up to 30 pounds
D. Appropriate Licensure and Certification Requirements
This position is responsible for assisting Care & Population Health Management staff with increasing engagement of members in PrimeWest Health’s care coordination and Focused Wellness programs. The Care & Population Health Management Engagement Specialist assists members with enrollment in programs and coordinates with care coordination and Focused Wellness staff. In addition, this position assists with linking members with accessing services and answers basic benefits questions.

Pay: $50,000.00 - $60,000.00 per year
