Job Summary:
The Patient Advocate will assist patients and their families in managing through the vast array of services and programs. The Patient Advocate will tailor services to the unique wellness needs of the patients served. The Patient Advocate will participate in health screenings, provide patient education, and offer appropriate referrals. The Patient Advocate will be responsible for cultivating relationships with patients and their families, physician partners, members of the healthcare team, and community partners. The Patient Advocate will be responsible for the collection of data related to contacts and referrals as outlined by the department manager. The advocate will be engaged in discussing strategies to continually improve patient and provider experience. The Patient Advocate will be involved in the identification and support of the development of programs and services to enhance patient care.
Duties:
- Serve as an advocate for patients/caregivers, ensuring they have the information, services, and resources needed.
- Assist in training and onboarding of additional caregivers.
- Provide holistic care support, including addressing physical, psychosocial, emotional, cultural, financial, and spiritual needs.
- Facilitate, organize, and coordinate clinical activities associated with the assigned clinical program.
- Develop, maintain, and report metrics for navigation services and programs. Track, monitor, and help prioritize allocated program funds as appropriate.
- Ensure health screening activities, including assessments, health profiles, vital sign checks, and lab draws are performed as needed.
- Act as a resource for staff and recommend staff education needs to the manager.
- Follow established protocols and procedures.
- Demonstrate responsibility and accountability for professional practice, including attendance, punctuality, and patient confidentiality.
- Maintain a positive public relation image and working relationships.
- Obtain necessary knowledge and skills needed to maintain competence and certifications.
- Complete all mandatory requirements, including annual safety programs, CPR, infection prevention, and annual physical.
Qualifications:
- Bachelor’s degree in Social Work.
- At least 1 year of experience in a hospital or clinical setting is highly desirable.
- Knowledge of disease processes, related treatment protocols, common side effects, and management.
- Ability to think critically, synthesize information, and communicate patient care needs in an organized and fluid manner.
- Ability to work independently and collaboratively, triage patients, and keep the care team informed of patient status.
- Basic computer skills: Microsoft Word, Excel, and Outlook.
- Ability to handle multiple tasks and appropriately prioritize.
- Strong oral and written communication skills.
- Ability to maintain patient confidentiality and privacy.
- Ability to work effectively with diverse populations.
- Ability to provide constructive feedback, work with teams, and teach staff and patients.
- Create and sustain respectful, inclusive, positive working relationships.
- Flexibility in adapting to new and challenging situations.
