Care Coordinator Needed (Adult Team)

The Care Coordinator (CC) supports adult mental health and/or medically compromised consumers by conducting home and provider visits. CC connects clients with severe mental illness to resources in the community, including housing, benefits, and basic needs.
Care Coordinators are responsible for working collaboratively with their clientele, all their support systems to include community providers to insure support for overall health and wellness.
ESSENTIAL ACCOUNTABILITIES:1. Works in a team environment to meet deadlines and needs of clients.
2. Carries an individual caseload of approximately 45 clients.
3. Discusses the program and services offered to the participant, from the point of intake to discharge.
4. Assures timely intakes, assessments, and referrals.
5. Is directly responsible for accessing care coordination services for the participant.
6. Coordinates all assessments from the various disciplines, developing case plans, and referrals for treatment, including, but not limited, health, mental health, substance abuse, social, vocational educational housing, and employment services.
7. Ensures that all participants receive quality program services and referrals to other community services within the policies and guidelines of the Care Coordination Program.
8. Maintains all case records in direct accordance with Care Coordination Program standards of practice.
9. Obtains all necessary records from hospitals, doctors, and other agencies, both past and present that were/are involved with the participant.
10. Follows up on all assessments and recommendations.
11. Is responsible for meeting with the participant on an as needed basis for follow up and counseling.
12. Conduct field-based community visits.
13. Prepare and revise care plans to reflect client needs and personal goals with a focus on maintaining health and wellness.
14. Complete all required monthly documentation as required to ensure appropriate and accurate billing.
In addition, CC meets weekly with the program director and participates regularly in trainings. CC is on call after hours on a rotating schedule.
Prior experience with a Care Management Agency a plus.

Requirements: Bachelor’s degree in social work, psychology, or related field and at least 4 years of experience in providing direct care to mental health consumers. Must possess excellent interpersonal, organizational, and time management skills. Must have outstanding communication skills, both verbal and written. Bilingual (Spanish) is preferred.
In addition, CC meets weekly with the program supervisor and participates regularly in trainings. CC is on call after hours on a rotating schedule.
