Title: APIP Senior Case Manager

Reports to: Director of ATI and Family Services
Salary Range: $50,000 - $55,000
Status: Full Time; Regular; Non-Exempt
Location: Long Island City
Days/Hours: Varies: Monday through Saturday, 9:00am – 5:00pm, 12:00pm – 8:00 pm. Subject to change depending on program needs
Position Summary:
The Senior Case Manager is responsible for facilitating Abusive Partner Intervention groups, perform case management duties and supervise Case Managers in their responsibilities.
Essential Duties and Responsibilities:

Supervise and guide case managers in their responsibilities;

Facilitate Abusive Partner Intervention groups using evidence based curricula;

Perform general case management duties, including weekly individual sessions with clients, referrals to community based services, developing client schedules, and client centered treatment plans;

Perform chart audits in accordance with program rules and protocol;
-
Ensure accurate and timely data entry of all client information into the agency’s database;

Assist clients in identifying and prioritizing needs;

Provide referrals to ensure all participants’ concrete needs are addressed;

Provide accurate and timely data and information to be included in court reports;

Monitor participants attendance and compliance with mandate requirements;

Conduct toxicology for clients as needed;

Participate in case conferences for optimum collaboration among departments/programs and external agencies;

Attend and participate in meetings, conferences, and trainings;

Engage in his/her own ongoing professional development; and

Additional responsibilities as required.
Qualifications:
-
High school diploma or GED/HSE required, some college preferred;
-
Experience and commitment to working with mandated participants with criminal justice involvement;

Strong Microsoft Office and database entry skills;
-
Strong assessment and engagement skills;
-
Strong written and verbal communication skills;

Bilingual English/Spanish a plus;

Strong organizational and leadership skills;
