Position: Case Manager
we are looking to add a full time Case Manager to our team. This position will provide rapid re housing and case management services to at-risk families in the child welfare system and support drop in clients seeking services from our food pantry. . Key responsibilities:
- Providing Advocacy to FRC walk in clients seeking food, hygiene, and other support services
- Providing case management to SPA 8 families in the child welfare system
- Completing the comprehensive standardized assessment
- Developing and implementing a housing and supportive service plan
- Conduct Housing and other Inspections as required
- Tracking financial assistance benefits and inputting information into HMIS/Clarity as needed
- Ensuring families have the resources needed to secure and retain permanent housing
- Making referrals to community based supportive services and coordinating with HIS programs and DCFS
- Ensuring client files are accurate and up to date
- Greet clients, volunteers, and donors to provide direction as needed
- Attending case conferences, trainings, and meetings as required and completing other duties as assigned
- Attend trainings as required by funders and those identified to enhance overall skills to help with service delivery
- Other duties as assigned including filling in where needed in the absence of front desk receptionist
Qualifications:
You have a Bachelor’s degree in social work or a related field, or 4 years of equivalent social services experience with an emphasis on case management.
- You have a background in substance abuse and recovery.
- MUST be Bilingual.
- You have the ability to listen with sensitivity to others’ feelings, needs, and points of view.
- You have the ability to demonstrate tact and courtesy while projecting a positivity.
- You are familiar with the Harm Reduction and Housing First models of service delivery.
- You have a valid California driver’s license and adequate auto insurance.
- You have a valid California driver’s license, a clean driving record with no more than two driving infraction points, and adequate auto insurance.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
Expected hours: 30 – 40 per week
