Staffing Coordinator Bilingual - Home Care Office Position
Pay rate will be determined during interview, Based on Experience.
Must have Weekend availability to work on call at least once a month
The Home Care Patient Coordinator is responsible for assigning agency para-professional and as needed to professional staff to patients. Responsible for maintaining ongoing communication with home care personnel (HHAs/PCAs), patients/their families, supervisor/s and insurance providers as needed for the daily operations of the Agency. Provides basic case management and coordination of services.
REGULATORY STANDARDS:
Adheres to the Agency Policies and Procedures when scheduling and assigning staff to cases.
Ensures all staff assigned meet compliance standards
PERSONNEL:
Participates in recruitment, hiring and orientation activities as needed.
Keeps record of aides' attendance and absences.
Participates in staff disciplinary process, as necessary.
Participates in annual administrative areas of performance evaluation for the HHA/PCA.
Receives and communicates customer/patient complaints to supervisor, as necessary.
PROFESSIONAL:
Schedules case coverage, always informing both patient and/or significant other of the name of the scheduled caregiver.
Participates in basic case management activities.
Maintains and updates scheduling lists.
Maintains and updates patient rosters, as directed.
Participates in verification of aides' attendance for payroll and program purposes.
Prepares statistical data, as requested.
Participates in agency On-Call.
Works directly with the nursing staff regarding staffing.
Attends meetings as requested.
Answers telephones and responds to inquiries by directing calls to appropriate staff
Other assignments as directed to facilitate daily operations.

Good organizational skills
Good telephone, organizational, typing and communication skills necessary.
Ability to maintain accurate schedules, notes and records.

Outlook, Word, Adobe Reader, Excel
Previous Home Care Experience and Knowledge of HHA exchange Preferred, but not required. Pay rates based on experience and will be determined during an interview.

Pay: $19.00 - $25.00 per hour
