SNF Physical Therapist Assistant or Physical Therapist needed to oversee the rehab department in a skilled nursing facility in Fond Du Lac

You are a PT/PTA who:
- Holds a current License/certification in Wisconsin

Job Duties Include:
- Act as liaison for assigned facility and assist with collaboration with the interdisciplinary team
- Provide ongoing facility updates to PRN team regarding staff/patient concerns, facility status, compliance updates, facility changes, facility conflicts or complaints, equipment needs, staffing, etc.
- Participates in the orientation and onboarding of clinicians, including procedures, competencies, documentation, and patient treatment.
- Coach/mentor clinicians to improve patient care, treatment strategies, documentation, time management/productivity, etc.
- Provide Physical Therapy treatments to patients according to treatment plans developed by the Physical Therapist
- Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient specific DME including mobility aides, wheelchairs, cushions, orthotics, and adaptive equipment.
Job Type: Full-time
Pay: $30.99 - $32.17 per hour
