The Health Case Manager (known at Exodus as the PHG case manager) will work to connect refugee clients to dental, medical, and vision services. The PHG case manager will work closely with other teams. This role is fast-paced in nature with a goal of providing short-term health case management services to clients. The PHG case manager will receive and process referrals, engage with clients, schedule appointments, support clients in accessing care, advocate for care when issues arise, and train clients on how to navigate systems effectively through use of Medicaid taxis and other trainings. The PHG case manager hosts training such as: monthly group trainings and/or one-on-one as-needed trainings to support clients in reaching self-sufficiency.
Some examples of a typical day may include scheduling appointments, calling clients with an interpreter to bridge the language barrier and make a plan to connect them to medical providers, sending Ubers to help clients get to appointments, accompanying clients to appointments, and training clients on how to navigate systems independently such as through Medicaid taxi trainings. Position Summary:
The primary purpose of the Health Promotions Associate (Extended Services) is to ensure that Exodus clients are connected to affordable and culturally/linguistically competent medical care. This position will assist all Exodus clients other than those in the R&P period. This position will achieve this goal by identifying and removing obstacles preventing clients from accessing care. This position will assist clients in navigating medical issues and problems with insurance and billing, and will generally serve as a source of medical case management support for Extended and RSS teams. This position will also focus on COVID-19 vaccination, messaging, and testing as needed. This position reports to the Manager of Health Services.
Responsibilities:
- Provide medical support to clients outside of the R&P program, including secondary migrants, clients who have completed their R&P period and need further short-term medical support, non-primary clients enrolled in RSS, and other qualifying populations.
- Assist clients in applying for and/or maintaining FSSA benefits for which they are eligible.
- Assist clients in addressing barriers to accessing health services, including interpretation, transportation, health insurance and benefits, making appointments, understanding provider instructions, medications, and responding to medical-related mail.
- Assist clients in scheduling medical appointments as needed.
- Help clients who are not enrolled in other medical case management programs to navigate medical bills.
- Arrange transportation trainings for clients, including Medicaid Cab, bus, Uber, Lyft, and any others that apply.
- Maintain programmatic knowledge of all medical services involving Exodus clients, including health insurance.
- Maintain up-to-date knowledge of CDC guidelines related to COVID-19 and communicate these to staff as needed.
- Assist with COVID-19 messaging to clients as needed.
- In coordination with other staff, ensure that clients have sufficient PPE.
- Conduct outreach to clients applying for LPR status or other groups in need of COVID-19 vaccination and assist them in obtaining the COVID-19 vaccine as needed.
- Assist clients in getting tested for COVID-19 and provide necessary follow-up in coordination with other staff.
- Attend weekly staff and team meetings.
- Participate in Exodus staff training and development activities.
- Participate in the emergency phone and airport rotation.
- Serve as a medical resource and support to members of the Extended and RSS teams.
- Perform other duties as may be required by the supervisor.
Position Criteria:
- Preferably have a minimum of two years of experience in social services.
- Must be able to effectively communicate with clients of diverse cultural and language backgrounds.
- Must be comfortable in intercultural environments.
- Must be detail oriented.
- Must be a self-starter with a high level of organizational skill, flexibility, and commitment to refugees.
- Must be able to function as part of a team.
- Should be familiar with the local social services community.
- Must have PC proficiency and be competent with Outlook, Word, Excel, and PowerPoint.
Job Type: Full-time
Pay: $21.00 - $22.00 per hour
