JOB SUMMARY:
Responsible for the coordination of all aspects of care and support to the patient/family and facilitates communication and interaction among personnel providing services to terminally ill patients.
QUALIFICATIONS:

1. Must be a graduate of an accredited School of Nursing. Requires current license as an LVN in the State of California. Requires documentation of a minimum of one year prior related nursing experience in an acute care setting.
2. Requires current CPR Certification or completion within (6) months of hire.
3. Minimum of (2) years hospice or home health experience preferred.
4. Knowledge of local, state, and federal laws for delivery of hospice services preferred.
5. Requires the demonstrated ability to relate effectively with physicians, health care professionals, the community, patients/families, and members of the hospice interdisciplinary team.
6. Requires ability to frequently stand, walk, reach with hand and arms, stoop, kneel, twist, and crouch.
7. Requires ability to effectively manage personnel; requires administrative skills to include but not limited to staff selection, development, motivation, scheduling, and evaluation.
8. Requires proficient ability to speak, read and write English.
9. Requires ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable; must provide proof of liability insurance.
10. Requires ability to maintain confidentiality.
11. Proficient working knowledge and ability to accurately and timely operate and perform computer related tasks with specific equipment and software applications required.
12. Requires sufficient good health to properly discharge duties. Employees shall not be permitted to work who have infectious disease or skin lesion for the duration of the communicability.
RESPONSIBILITIES/TASKS:
1. Demonstrate knowledge of medication administration, interactions and side-effects.
2. Demonstrate knowledge and competence in pain and symptom control concepts and ensure interventions are carried out on a timely basis.
3. Ensure the case management approach for referral to other services as needed.
4. Prepare and submit clinical and progress notes that demonstrate progress toward established goals according to policy and procedure guidelines.
5. Communicate information using current processes/available technology on a timely basis.
6. Provide specialized hospice training to other staff, family members and informal caregivers to ensure adequate patient care.
7. Ensure on-going evaluation of the patient/family response to care.
8. Assess the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care provision.
9. Evaluate own needs for support and identify systems to meet the needs.
10. Assume responsibility for reordering patient medications/maintaining inventory of supplies.
11. Demonstrate knowledge and awareness of cost containment by economical use of supplies, medications and other resources.
12. Ensure patient privacy when providing care and treatment.
13. Participate in clinical call as scheduled.
14. Ensure compliance with Death Pronouncements as per agency protocol.
15. Complete audits of discharged patient charts within 30 days of discharge.
16. Participate in the QAPI process.
17. Monitor self, staff and patient safety in care setting.
18. Participate in departmental and other meetings as requested.
19. Participate in all required In-Service Programs.
20. Maintain current level of skills and evidence of competency.
21. Adhere to all Confidentiality policies and procedures and maintain confidentiality of administrative information.
22. Develop and update the POC as necessary
23. Attend IDT as required
24. Apply specific criteria for admission and recertification of hospice patients.
25. Assess risks for grief, special cultural or speech needs.
26. Keep the team informed of the patient’s condition.
27. Maintain compliance with all Faith and Hope Hospice policies, procedures and requirements.
28. Maintain compliance with all state and federal laws and regulatory requirements.
29. Work successfully as part of a team and responsible for developing then maintaining strong working relationships with co-workers.
30. Attend and participate in staff meetings and training as required.
31. Perform general office tasks as required.
32. Perform special assignments, projects, and other duties as required.
