LSW, LISW, LPC, LPCC
· Provides direct services to clients with SUD and MH disorders via assessment, individual, and group facilitation using an evidenced based curriculum and treatment model.
· Collaborate with Aftercare Coordinator regarding discharge planning and placement.
· Participate in scheduled individual and group supervision as required by Cedar Oaks and state law requirements.
· Participate in a team environment, and daily census meetings.
· Proficient in all clinical documentation, including (but not limited to) biopsychosocial assessment, individual progress reports, individual weekly progress reports, group notes, treatment plans, releases of information, and family notes.
· Participate in a weekend and holiday rotation for inpatient programming.
· Maintain client confidentiality, follow all HIPPA/42 CFR Rules and other state and federal guidelines.
· Proficient at EMR documentation.
