We are looking for a Nurse Practitioner to join our practice. In this position, you will be working with patients in post acute care facilities Skilled Nursing home, LTAC and Rehabs addressing their health needs, providing care with good knowledge on Medication management. You will also be required to maintain good communication with nurses and administration staff while performing as part of a treatment team. Willing to educate patients on preventive health practices and the treatment of illnesses. You’ll partner with our physician and fellow healthcare professionals to treat patients with various medical conditions and disease processes.
Requirements -
2-3 years' experience in the NP role
3 plus years as RN in an acute or post-acute facility
Below are some of the responsibilities as part of Providing care to the patients
- Quality treatment with good application of medical knowledge
- Communicating with nurses daily and answer questions appropriately
- Avoid errors in giving medication and treatment
- When in Doubt need to double check with physician
Answer the calls/texts from nurses as needed.
Active Communication with Physician and patient families
- Maintain calm and composure when talking with nurses, staff, patients and their families
- Take extra time in explaining to patients when needed and any disagreement with patients, nursing or administration staff needs to be reported with doctor
With every other weekend (Alternate weekend and half long weekends) as working days the patient load may vary (higher or lower) depending on situations
Documentation of notes on same day and finishing up pending medical records
Submit the billing sheet with updated details (ICD and EM codes) at the end of the day
Occasional need P2P communications with insurances for patient approval to next level of care
Proper facilitation of discharge process involving active communication with Case managers
