JOB DESCRIPTION SUMMARY
Rivercross Hospice is currently seeking a full-time Registered Nurse Case Manager. We are a locally owned, non-corporate Hospice with office locations in Bartlesville, Muskogee, Pryor and Tulsa. Rivercross is looking for a Nurse Case Manager that can cover Tulsa, Okmulgee and the Cleveland area. The Hospice Nurse is a registered nurse and an Interdisciplinary Team Member whose primary responsibility is to present Rivercross services and philosophy to patients and families in a manner that result in the admission of appropriate patients. Responsible for coordinating hospice services for managed care patients which includes establishing and maintaining the patient’s plan of care, establishing and maintaining positive relationships with managed care customers and related referral sources, and negotiating service agreements.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Completes an initial, comprehensive and ongoing comprehensive assessment of the patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
- Provides professional nursing care by utilizing all elements of nursing process.
- Assesses and evaluates patient’s status by: o Writing and initiating plan of care o Regularly re-evaluating patient and family/caregiver needs. o Participating in revising the plan of care as necessary.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan that established goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Facilitate the patient’s efforts toward optional comfort care.
- Maintains comprehensive working knowledge in the field of managed care and shares information with appropriate organization personnel.
- After hours On-Call as needed.
- Perform other duties and activities as delegated by the Patient Care Manager.
- Performs Admits as needed to ensure patient care and staff utilization

POSITION QUALIFICATIONS
- Current RN license.
- Vaccinated against Covid-19
- At least three (3) years’ experience in health care and managed care coordination preferably in hospice or similar operation.
- Complies with acceptable professional standards and practice.
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
- Possess excellent critical thinking skills.
- Team development skills to work as part of the team concept.
- A passion for working with Hospice and helping patients and families to have a quality end of life experience.
- Excellent clinical, end-of-life case management skills, with a strong background in comfort care.
- Excellent observation skills, good nursing judgment, verbal and written communication skills, problem solving skills, basic math skills and nursing skills per competency checklist.
- Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients.
- Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
- Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Current CPR certification.
- No weight restrictions
- Able to drive at dusk or night
Job Type: Full-time
Pay: $66,000.00 - $80,000.00 per year
