Job Summary:
The MDS Coordinator is a Registered Nurse (RN) who is responsible for the overall supervision, coordination, maintenance, and timely completion of the MDS Assessments, Resident Care Plans, and all relevant audit tools and documentation related to the MDS process. The MDS Coordinator oversees the facility assessment and quality measures program and ensures assessment and quality compliance in accordance with facility policy and regulatory requirements.
Salary Range: up to $41 an hour.
Responsibilities:
- Supervises assessments and care plans and participates as needed in providing care for residents in accordance with nursing standards of care, facility policy & procedure, and pertinent regulations.
- Responsible for ensuring that nursing personnel complete their portion of MDS assessments as assigned, in accordance with facility standards and regulatory requirements.
- Responsible for ensuring that nursing personnel complete the timely documentation of nursing care, administration of medications, physician orders, and all other aspects of care required for documentation in the MDS assessment.
- Coordinates with Nursing, Social Services, and Therapy to ensure appropriate planning for discharge, transition, etc.
- Actively participates in the Quality Assurance by tracking and analyzing data, maintaining logs, and implementing plans of action as indicated.
- Other duties as assigned by Director of Nursing.
Qualifications:
- RN license required
- Three years of MDS experience, preferably in a long-term care setting.
- Thorough knowledge of clinical nursing and SNF Regulations, as well as practical knowledge of nursing skills and standards.
- Thorough knowledge of the MDS Assessment Requirements, Care Planning, and other aspects of the MDS role.
- Good communication skills
- Ability to use initiative and good judgment in determining the needs of both staff and residents.
