Job Summary:
We are seeking a compassionate and skilled CNA to provide exceptional patient care in a home setting. As a CNA, you will be responsible for delivering personalized medical care to patients of all ages, including pediatrics, seniors, and individuals with behavioral health needs. Responsibilities:
- Provide comprehensive patient care in accordance with established medical protocols and individualized care plans
- Administer medications and treatments as prescribed by healthcare professionals
- Monitor and record vital signs, ensuring accurate documentation
- Assist with activities of daily living, including personal hygiene, mobility, and feeding
- Conduct patient assessments and collaborate with the healthcare team to develop and implement effective care plans
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Perform catheter care and other specialized procedures as required
Skills:
- Proficient in providing patient care across various age groups
- Strong knowledge of medical terminology and procedures
- Ability to assess patient needs and develop individualized care plans
- Competent in monitoring vital signs and recognizing changes in patient condition
- Skilled in administering medications and treatments accurately
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals
