Summary of Position:
The purpose of this position is to provide both clinical and clerical support services in a setting that delivers care to patients. The Medical Coordinator II assures the continuity of client care, providing an essential link for communication involving all team members. Assists in the examination and treatment of patients under the direction of the physician.
Essential duties:
Duties include, but are not limited to:
- Outreach Medical Coordinator II will support a provider in the Neighborhood Partnerships and/or Street Medicine teams.
- Collaborates with the physician throughout each shift to determine work assignments and priorities, as well as communicate the status of delegated task completion and outcome of patient data findings.
- Responsible for collecting, collating, and entering required data and information into the applicable database or format in an accurate and timely.
- Treats each patient in a manner that preserves and supports patient confidentiality, patient rights, and patient dignity.
- Checks inventories on a weekly basis and orders supplies.
- Facilitates patient transportation as needed.
- Ability and willingness to perform medical assistant duties at Neighborhood Partnerships and /or Street Medicine sites.
- Utilizes methods to increase provider efficiency, continuity of care, and access for patients.
- Monitors front and back-office tasks to ensure standardization and adherence to processes and procedures.
- Collaborate with outreach teams to provide comprehensive training for clinical support staff, ensuring the training model is patient-centered and mission-focused.
- Assist with monthly medication and quality control audits.
- MC II Daily Tasks: Collect, verify, register new patients, scan records to chart, process referrals, call for appointments, request medical records, obtain vitals, administer vaccinations and medical injections, basic wound care, call on prescriptions, draw blood, process lab specimens, maintain clinic inbox.
- Drives the Mobile Medical Unit (MMU) or Circle the City vehicle to support team.
Qualifications:
Basic Knowledge and Skills:
- Basic knowledge of medical terminology.
- Certified Clinical Medical Assistant license.
- Able to obtain vital signs accurately.
- Ability to use a personal computer for recording patient data /history.
- Proficient in reading and writing skills in English.
- Excellent communication and customer service skills.
- Ability to work in a fast-paced, challenging environment.
- Demonstrated interest in working with an underserved population.
- 2 years of MA experience.
- High school diploma or GED required.
- Experience in a medical environment is highly desirable.
- Bi-lingual communicative skills necessary.
- Basic Life Support (BLS) certification.
- Requires a current Arizona state driver’s license.
- Must be at least 21 years of age.
Physical and Mental Requirements:
- Position requires extended periods of sitting and standing.
- Position requires regular bending and reaching, including transfer of patients.
- DOT physical exam.
