Harmony Care Services
Harmony Care Services offers support and assistance to individuals with behavioral disabilities. These disabilities can include a wide range of conditions, such as developmental disabilities, intellectual disabilities, and mental health disorders. We often work with individuals to develop and implement treatment plans that address their specific needs and goals. These plans may include therapies, counseling, and other types of support to help individuals improve their communication and social skills, manage their emotions and behaviors, and achieve greater independence and self-sufficiency. We may work in a variety of settings, including schools, community centers, and residential facilities, and may also provide support to families and caregivers.
We are looking for a compassionate Part-time Registered Nurse or LPN with experience providing care for individuals with intellectual disabilities to join our home health team. The successful candidate will be responsible for providing care to patients in their homes, assessing patient needs, developing and implementing care plans, and monitoring patient progress. The ideal candidate will have excellent communication skills, a commitment to patient safety and comfort, and the ability to work independently with minimal supervision.
1. Must be a Registered Nurse or LPN licensed in the State of Indiana in good standing.
2. Minimum of three months of nursing experience.
3. Certification as a trainer and/or the ability to receive and maintain trainer certification in Core A & Core B Medication Administration, and CPR & First Aid.
Responsibilities:
- Perform home health assessments and develop care plans.
- Provide patient education and support.
- Administer medications and treatments as prescribed by the physician.
- Monitor patient progress and document changes in condition.
- Coordinate care with other healthcare providers.
- Assist with activities of daily living (ADLs).
- Maintain a safe environment for patients.
GENERAL REQUIREMENTS AND WORKING CONDITIONS
1. Must provide own transportation, have valid driver’s license and driving record must meet insurability requirements.
2. Must maintain and provide documentation of auto insurance at limits set by agency.
3. Must have ability to lift/move individuals or materials weighing up to 75 lbs. periodically.

Pay: From $25.00 per hour
Expected hours: 15 – 25 per week
