Position: Care Coordinator | CMA
Reports to: Director of Medical Services
Job Description:
The Care Coordinator is a Certified Medical Assistant and is responsible to perform tasks associated with Health Care Coordination for our Guests’ medical needs. The Care Coordinator provides direct medical care and assists with Activities of Daily Living (ADL) care provided by other Guest staff. The Care Coordinator is responsible for record keeping for all medical aspects of Guest care. The Care Coordinator will work with other IADC medical staff on Guest Health Care Coordination (HCC) and will schedule, attend and provide transportation for Guest Medical appointments. The Care Coordinator may participate in non-medical Guest care such as outings and activities.
Essential Functions:
- Assist with administrative duties as it relates to assessments of current and prospective Guests.
- Carry out tasks associated with the medical plan of care that are within his/her scope of practice.
- Completion and documentation of regular monitoring of vital signs/weight/blood sugar, medication administration, bowel and bladder care, and education.
- Update and maintain Guest medical files, including medical plan of care, care records, and progress notes.
- Assist Guests and supervise Guest staff in assistance with ADLs, including eating/restorative feeding, ambulation, range of motion, transfers, toileting and showering.
- Ensure accurate record-keeping of ADL care.
- Document conditions, reactions and responses of Guests as related to assigned duties, and report to Director of Medical Services and/or Center Executive Director.
- Assemble, disassemble, and clean equipment and accessories.
- Maintain a safe environment at all times.
- Maintain confidentiality at all times. Ensure Guest privacy and dignity in all manners of care.
- Ensure security of all Guest medication stored and administered at the Center. Document medication administration.
- Assist with the maintenance of licensure and accreditation standards, including participation in the outcome measurement program.
- Collaborate with other IADC medical staff on Guest Health Care Coordination, including medical record consolidation, medication management, and medical appointment scheduling and attendance.
- Assist in keeping bathing room, shower room, and exam room neat and clean. May be required to perform heavier cleaning/mopping of all Center areas.
- Assist with, attend, and/or lead non-medical Guest care including activities and outings, as needed.
- Ensure toileting, feeding, and bathing needs are met, as required by specific guests.
- Assist with training and onboarding of new Guest care staff.
- Ensure all Guest care staff know how the plan of care for each Guest current and new admissions.
- Assist with completion of incident/accident forms as required when needed.
- Other duties as needed.
Qualifications:
- Certified Medical Assistant licensure
- Direct care experience working with geriatric and/or adults diagnosed with Intellectual Disabilities preferred
- Valid driver's license in state of residence
- Qualifying criminal background and driving record
- Certification/experience in certain areas of training including but not limited to CPR, First Aid, non-violent physical interventions, behavior management, medication administration, and driving
- Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations
- Ability to react to situations, i.e., rescue, preventing a resident from hurting themselves
- Ability to work with frequent interruptions

The employee must be able to use general office equipment such as computers, photocopiers, telephones, etc. The employee must be able to drive a car and perform housekeeping duties such as cleaning, cooking, snow shoveling, etc. The job may require exposure to outside weather conditions while working with Guests.
