Qualifications:
- Graduate of an approved Licensed Vocational Nurse program and licensed in the state of practice required.
- Minimum of two (2) years nursing experience in skilled nursing facility preferred.
- Knowledge of care plan process.
- Strong attention to detail, documentation, and resident assessment skills.
What it takes to be a MDS Nurse:
The MDS Nurse will assess, monitor, and document patient care, ensuring that documentation of resident care plans meet federal, state, and certification guidelines.
Responsibilities:
· Coordinating preparation of MDS and care plan as per regulatory requirements.
· Participating in the ongoing evaluation of documentation required for Medicaid and Medicare reimbursement.
· Assist the facility in assuring adherence to federal and state regulations and certification
· Actively participates in the regulatory or certification survey process and the correction of deficiencies..
· Report trends from completed audits to the Quality Assurance Committee.
· Collect data for each resident and interviews staff and residents as necessary to assure good standard of practice and as instructed in the MDS 3.0 User manual.
· Facilitates accurate determination of the Assessment Reference Date that accurately reflects the patient’s care needs and captures all resources utilized to ensure appropriate payment by Medicare/Medicaid and insurance programs.
· Maintains confidentiality of necessary information.
· Promotes positive interactions with internal and external customers, visitors, residents, and team members.
· Provides excellent customer service that supports resident care.
· Performs other related duties as assigned or requested.
· Participates in the identification of staff educational needs.
· Conducts self in a professional manner and in compliance with facility policies.
Physical Requirements:
- Prolonged periods of standing and walking.
- Must be able to lift up to 75 pounds at times.
Job Type: Part-time
Pay: $35.00 - $38.00 per hour
