JOB SUMMARY:
The Registered Nurse (RN) assumes the responsibility for application of nursing and clinical processes and delivery of quality patient care at our clinics under the supervision of Nursing Manager or Director of Nursing (DoN) and has accountability of the nursing process and the delivery of efficient patient care as defined in the Registered Nurse (RN) Scope of Practice. The RN supports patient evaluation, and carries out related nursing intervention, supports nursing patient evaluation, efficient system applications and recording of patient care/behavior. Works collaboratively with clinic interdisciplinary team members, medical staff, clinical operations, and with the Administrative/Operational Leadership teams to ensure achievement of patient, clinical or program goals as appropriate. Provides clinical care and demonstrated clinical competency for area of specialty patient populations. Application of and knowledge of regulatory requirements in delegation of tasks and in documentation. Assist in the development and implementation of quality, cost-effective, efficient systems and protocols for standardizing processes of patient care at clinics. Nursing job duties are to comply with the Nurse Practice Act, Title 22, Joint Commission (JC), CMS 42 CFR Part 460 regulatory requirements as appropriate. The plan for provision of nursing care to patients aligns with the company Code of Conduct, professional and educational standards, requirements as needed. The Registered Nurse works in the team-based model of care.
RESPONSIBILITIES
- Manages the care of patients in collaboration with other health professionals and members of the health care team in order to make decisions about treatment and attain health goals.
- Performs clinical patient care nursing duties within the scope of practice for Registered Nursing, with high integrity, quality, and meet nursing or population care standards.
- Performs screening, triage, assessment and/or patient intake for patient as required by job duties or responsibilities.
- Utilizes patient medical information, conducts nursing assessments, evaluates current treatment plan, (which may include medications, therapies, devices and/or interventions) to:
- *Prepare for and/or provide care based upon patient visit or encounter needs. *Obtain and evaluate diagnostic results as needed, i.e. vital signs, TB skin testing, *Evaluate and/or provide patient/family teaching or skill validation i.e. fall prevention, Performs therapeutic point of care diagnostic testing as ordered for patients, i.e. Blood Glucose, UA,
- Performs nursing procedures utilizing evidence based practice guidelines or clinical standards. Examples: medication administration, IV therapy, wound care, positioning, personal care,
- Maintains acceptable standards of nursing care based on SOS, community, JCAHO and applicable governing regulatory Agency requirements.
- Participates in and supports on-going efforts to ensure infectious diseases control program is applied for all clinic experiences.
- Communicates, coordinates and consults with members of the health team while performing nursing responsibilities.
- Assist in the development and implementation of quality, cost-effective, efficient systems and protocols for standardizing processes of patient care at clinics.
- Contribute to performance appraisals by providing feedback to the Nursing Manager or DON, about MAs and LVNs on her/his team
- Participate in MA and LVN clinical competency evaluation.
- Responsible for indirect oversight of all nursing as assigned in the clinic.
- Complies with federal, state, and local regulatory requirements and participates in the preparation and conduction of clinic or program regulatory audits.
- Responds to all medical emergencies and follows process outlined by SOS Emergency procedures.
- Required member of the clinic interdisciplinary team and participate in team based care meetings for care coordination, daily operations, care planning and service delivery.
- Maintains acceptable standards of nursing care, based on community, JCAHO and SOS standards.
- Works collaboratively with Clinic Leadership in planning and implementing medical services.
- Assist with monitoring of quality and compliance with all nursing standards (including, but not limited to, audits for EHR nursing documentation and clinic audits.)
- Follow up on Registry patients (Abnormal Paps, Mammograms, Colorectal, and Lab Results) by determining appropriate next actions and contacting patient to provide information and scheduling assistance as necessary.
- Track patients with a history of abnormal registry results to ensure compliance with recommended actions.
- Schedule patients with specialty providers as needed.
- Follow-up on in-basket message and management tasks.
- Perform all other related duties as assigned.
ADDITIONAL RESPONSIBILITIES
- Participates in orientation and development of new clinical nursing staff or students as assigned.
- Supports Nursing Manager with schedules within clinic for nursing support staff (RN,LVN & MA's)
- Maintains regulatory compliance of all back office/nursing activities (i.e. Logs, QC’s, and Nursing Stock Meds) and may oversee compliance at designated sites as directed by Supervisor.
- Follows up with Care Teams for patient flow.
- Assists the Nursing Manager and CBOM in preparing for regulatory/quality audits.
- May drive to trainings or other work sites.
- Attends staff and clinic meetings as requested.
- Helps nursing staff with questions, issues or patient-related conflicts.
EDUCATION/QUALIFICATIONS:
- Associate Degree required; BSN preferred
- Accredited school of nursing graduate with a current California State Board of Nursing Registered Nurse License required.
- Minimum of one year experience as RN in clinic setting strongly preferred; proficient clinical skills
- Experience in a primary care or ambulatory care environment i.e., clinic, FQHC or Urgent Care preferred.
- CPR/BLS certification
- Communicates well in oral and written format
- Bilingual in English and Spanish required
- Works successfully and respectfully with patients and clients of diverse backgrounds and cultures; mentally ill, substance abusers, and homeless patients
- Knowledge of the Patient Centered Medical Home model of care
- Successfully completes clinical competency validation including special care and management of equipment
- Achieves proficiency with EHR within three months of training
- Uses Microsoft Word, Microsoft Excel, internet, and Practice Management Systems efficiently; accurate keyboard skills
