Seeking a Nurse for a position with Smoky Mountain Home Health and Hospice in the Newport, TN area. Smoky Mountain is a locally owned home health and hospice company with 7 offices providing care to patients across East Tennessee.
Qualifications: Licensed in Tennessee as a Registered Nurse.
Job duties include:
- Conducts an initial evaluation and completes the hospice and/or home health admission process for all appropriate patients.
- Conducts a physical assessment of the patient.
- Must be able to lift 50 lbs. or greater.
- Explains nursing and other hospice or home health services available to patients and families as a part of planning of care.
- Develops, implements and updates the nursing care plan and provides skilled nursing care as outlined in the nursing care plan.
- Provides treatments and procedures requiring substantial specialized skill in accordance with the Physician's order.
- Teaches, supervises and counsels the patient and family member(s) regarding the nursing care needs and other related problems of the patient at home.
- Evaluates the effectiveness of the nursing service to the patient and family.
- Consults with the attending Physician concerning alteration of plan of treatment.
- Maintains dialogue with the Physician as well as other people in the community who provide specific assistance to the patient, including community resources.
- Teaches, supervises and counsels patient and family member(s) regarding the nursing care needs and other related problems of the patient at home.
