Hospice Services of Lake County is seeking a full-time RN Case Manager who will make a meaningful impact in the lives of our palliative and hospice patients receiving symptom management. The successful candidate will collaborate with interdisciplinary team members to manage symptoms, provide education, and coordinate support for the patient and caregivers.
Specialized training, coaching, and support are provided as needed for clinical team members.
The Registered Nurse Case Manager plans, organizes, and coordinates care under the physician’s orders with other Interdisciplinary team members. Care and family support are provided in the patient’s place of residence whether it is a private home, a residential care facility, or a skilled nursing facility.
Work hours are Monday-Friday pm from 8:00 am-4:30 with an occasional evening on-call coverage requested with a one-in-6 Saturday daytime schedule. A 32-hour work week is also allowed.
Current California RN license and a minimum of three years of experience preferred. Valid driver’s license, reliable transportation, drug screening, and background check will be required.
