Floral Home Care located in Queens county is looking for experienced Intake Coordinator.
The ideal candidate will be motivated, enthusiastic and able to work closely with both our Marketing and Coordination teams to facilitate the overall growth of our client base. Key responsibilities include processing referrals and admissions, as well as communicating directly with clients and vendors to ensure customer satisfaction.
The Intake Coordinator is responsible for for the following but not limited to:
-Registering, and admitting patients at the facility. Creating patient records. Make appointments and schedule procedures for future medical treatments.
o Accepting incoming referrals and entering them into tracking software.
o Managing the process of converting a referral into an active home care case.
o Addressing inquiries via phone and e-mail, and obtaining all medical, demographic and insurance information needed.
o Ensuring referral forms are completed correctly, and assisting with securing any supporting documentation needed.
o Promptly advising appropriate staff of new admissions, and entering the cases into scheduling system.
o Maintaining accurate, current records of admissions, discharges and other changes to the caseload.
o Acting as liaison with family members and referral sources
o Acting as a point of contact with contracted vendors.
Requirements:
o Excellent written and verbal communication skills.
o Knowledge of the different facets of home care.
Qualifications:
o At least 1 year of experience in Home Care Agency.
o Bilingual a plus but not required.
