Job Title: **Case Manager – Homekey
Reports to: Program Manager
Status: Full-Time, Non-Exempt

SUMMARY
The Case Manager will be working out of a designated Project Homekey Site. The Case Manager’s Primary responsibility are to outreach and engage new clients, coordinate housing, support clients with mental and primary health recovery services, and ensure community integration. The person in this position will use harm reduction model to help maintain the highest quality services and foster collaboration between clients and programs. This person is required work a 40-hour workweek and may find it necessary to work some evenings and weekends to meet with clients. This position calls for additional support in other Orange County areas if workflow deems it necessary.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Maintain a caseload of clients for case management services
- Engage individual to determine motivation and eligibility for housing
- Complete intake packet with referred members upon project enrollment
- Enter notes in HMIS within 24 hours of client contact and document all activities
- Collaborate with members to develop a savings budget for clients and housing goal plan to determine eligibility for housing opportunities; review and update plan quarterly and upon completion of goals
- Work with members to complete a biopsychosocial assessment
- Assist members in obtaining identifying documents such as ID, Social Security Card, Birth Certificate and other required HMIS documentation
- Participate in CES Match Meetings to match program participants to appropriate housing opportunities
- Assist clients to find a safe place to live and contact property managers and landlords to set-up appointments to show clients different apartment options
- Develop effective, trusting relationships with clients, with a focus on facilitating independence and maintenance of improve physical and mental health
- Coordinate services with partner agencies as needed
- Assist members with enrollment in government services including Medi-Cal, GR, SSI, SSDI, SNAP and others when applicable
- Responsible for tracking clients’ progress from enrollment to stabilization
- Assist with tasks pertaining to facilitating daily groups, setting up workshops, and other programs
- Maintain client files with vital information (i.e. SSI papers, proof of income, court documentation, etc.) Update client progress on bi-weekly basis to ensure that the needs of participants are being met in a timely manner
- Track program participation, employment and compliance and notify Program Director of compliance issues when needed
- Coordinates all services required by clients to complete program
- Transport members to complete various housing tasks such as viewings, lease signings, and housing documentation (SSI, GR, etc.)
- Perform daily room-checks and client check-ins
- Work with housing authority and other housing entities to advocate on clients behalf
- Maintain professionalism during all contact via email, phone, text and fax etc. ensuring HIPPA compliance
- Perform monthly file audits
QUALIFICATIONS
- Current, valid California Driver’s with an acceptable driving record and reliable vehicle
- Able to transport clients in company vehicle
- Bachelor’s degree from an accredited college of university in Psychology, Social Work, Human Services or a related field OR 2 years’ experience providing services to the homeless, highly vulnerable populations.
- Minimum of one-year full-time experience or internship working with the homeless, preferably those living with mental illness and/or substance use disorder
- Detail oriented with excellent time management, organizational, communication, interpersonal, and computer skills
- Able to work in a high tolerance Harm Reduction model with members who have multiple barriers while providing self-determination
- Able to learn quickly and work effectively with a wide range of constituencies with minimal supervision
- Strong interpersonal and team building skills
- Self-starter with the ability to stay ahead of the curve and thrive in fast-paced work environment
- Able to obtain and maintain CPR/1st Aid certification
- Experience using HMIS
- Able to complete SOAR certification

Job Type: Full-time
Pay: $21.00 - $25.00 per hour
