JOB SUMMARY:
Stable and growing, CHAP accredited Home Health Agency is seeking a dynamic candidate to join our team as PRN to complete assessments in the home. This is a position where you drive from one home to another. We cover from Sarasota to Fort Myers.
Currently this position is primarily office-based as there is only a light case-load of non-skilled, field assessments.
ESSENTIAL FUNCTIONS:
- Ensures compliance with all federal, state, and local regulating bodies, relating to clinical aspects.
- Ensures compliance with CHAP Accreditation and is the acting Compliance Officer.
- Remains abreast of all changes and updates on Home Health/Private duty regulations and standards.
- Review patient referrals, works with Scheduler on referrals of high-risk cases prior to start of care, and communicates on patient satisfaction/complaints.
- Ensures the accuracy and completion of all patient chart documentation.
- Maintains and reviews patient satisfaction, ensuring patient satisfaction, making any accommodations as needed.
- Manages and enforces completion of annual compliance training and company policies and procedures.
- Maintains and supervises all clinical functions while maintaining complete compliance with federal and state regulations.
- Performs patient assessments and coordinates patients’ services by reviewing plans and frequency of care and treatments, clinical documentation, and progress notes.
- Communicates with case managers, hospitals, skilled nursing facilities and families regarding the client’s status of care.
- Maintain and lead Quality Assurance and Infection Control Programs.
- Implement and maintain company policies and procedures that are congruent with CHAP accreditation requirements.
- Monitors job performance through clinical performance evaluations and in-home supervisory visits.
- Works with Administration to meet or exceed revenue goals.
- Perform other tasks and assignments as directed by Administration.

EDUCATION / SKILLS / ABILITIES / AVAILABILITY
- College degree in nursing (combination of any college degree, plus experience)
- Valid Registered Nurse licensure in the State of Florida
- Two (2) years of work experience in the medical field (preferably a Home Health setting)
- Ability to make recommendations to address and resolve issues.
- Ability to perform educational training to internal and external staff.
- Ability to effectively deal with diverse groups/individuals and to maintain professional working relationships.
- Ability to organize workload, set priorities and meet deadlines.
- Be proficient in preparing and submitting correspondences and forms.
- Ability to operate office equipment, such as basic computer functions, printers, copiers/fax machines. Knowledge of modern office practices/software technology such as Microsoft Suite/Practice Mgmt Software.
- Knowledge and skills in records administration and maintenance.
