Care Navigation Center (CNC) Documentation Specialist organizes, communicates and documents information related to various hospice certifications according to state and federal regulations.
Position Details
- Work Location: In-office position; no work from home at this time
- Hours: Monday - Friday, 8:00am - 5:00pm
- Compensation Details: Base salary ($20.00/hour); full benefits package including 23 days PTO annual
Job Responsibilities
- Audit hospice patient admission certifications received from various physicians and nurses; input patient data and information into Suncoast EMR system
- Obtain attending/referring physician and/or medical personnel signatures on various hospice admission certifications; ensure consent documentation are promptly signed and accounted for.
- Update patient charts with signed hospice certifications as needed.
- Coordinate with other specialist team members regarding certification procedures and work loads
Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
- High School Diploma or equivalent, minimum
- One-year experience in an office environment, minimum. Experience in a health care environment preferred
