JOB SUMMARY:
Functions under the collaborative practice agreement with a physician, provides professional medical services in the community health center setting.
ESSENTIAL DUTES:
- Provides comprehensive examination, screening, health education within the scope of practice using a systematic approach to history and physical appraisal of the client.
- Develops and implements client comprehensive management plans, records progress notes, and assists in provision of continuity of care.
- Instructs and counsels clients regarding compliance with prescribed therapeutic regimens.
- Establishes a collaborative relationship with other medical providers and specialists; determines need for consultation and assists in medical care treatment provided at the direction of other specialists.
- Documents and maintains client records of services provided according to program standards.
- Participates in education of graduate nursing students as assigned.
- Will serve as the Program Manager ensuring planning, organizing, monitoring, and managing the day-to-day processes and operations of assigned program.
- Other duties as assigned.
QUALIFICATIONS:
- Graduation from an accredited nurse practitioner program. Master’s degree in nursing with advanced skills in physical assessment. ANA Certification prior to or within one year of employment.
- License to practice in the State of Florida plus current DEA registration.
PHYSICAL DEMANDS/WORKING CONDITIONS:
- Bending, lifting, reaching and walking.
- Exposure to clients, including clients who are HIV positive, possible Tuberculin and/or Hepatitis exposure (vaccination recommended).

KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge and skills for the provision of indirect client care in the practice of primary health care.
- Knowledge of the state and federal laws pertaining to client care and to community health centers.
- Knowledge of the structure and function of community health centers.
- Skill in communicating effectively with clients and their families.
- Skill in establishing and maintaining effective working relationships with other employees, clients and the general public.
RESPONSIBILITY FOR CONFIDENTIAL MATTERS:
- Requires constant knowledge of and access to sensitive medical and personal information of clients. Expected to follow AGAPE’s confidential procedures as noted in the Human Resources Policies and Procedures manual.
EFFECT OF ERRORS:
- Position must adhere to appropriate clinical procedures and protocols as mandated by City, State, and Federal Authorities. Errors could expose the Center to citations, fines, legal liability, and possible embarrassment.
HOURS OF WORK:
40 hours per week and as needed, including late evenings and weekends

Pay: $99,000.00 - $110,000.00 per year
