Job Summary:
The scheduler's main function is to schedule patient services and coordinate with staff.
Duties and Responsibilities:
· Schedules and appropriately documents patient schedules promptly.
· Handle telephone calls from clients
· Handle client concerns and complaints and refers to the appropriate individual
· Assigns field staff schedules
· Participates in arranging appropriate in-services for field staff.
· Assigns field staff schedules
· Maintain and reviews active clints and field staff frequencies
· Prepare for Interdisciplinary Group meetings
· Participate in coordinating care with management and patient Interdisciplinary Group/Team.
· Provide effective communication to patients, caregivers/family, staff members, other healthcare professionals and referral sources.
· Respond to the hospice and patient needs in a professional and creative manner.
· Demonstrate commitment, professional growth and competency.
· Promote the hospice philosophy and administrative policies to ensure quality of care.
· Maintains daily communication with assigned supervisors.
· Perform administrative duties to support the department.
· Review, completes and submits reports as requested.
· Perform general office duties as required of client services and field staff overview.
· Demonstrate current knowledge of principles of effective customer service.
· Participate in the Quality Assessment and Improvement Program.
· Fulfill additional performance responsibilities as assigned and as necessary
Qualifications/Experience:
Education: High school graduate. Graduate of an accredited school of vocational/practical nursing.
Licensure: If an LVN/LPN must have a current state license as a licensed vocational/practical nurse. Current driver’s license.
Experience: One (1) year of experience as a licensed vocational/practical nurse in hospice preferred.
KINNSER EXPERIENCE PREFERRED, FAMILIAR WITH MEDICAL TERMANOLOGY
Skills: If an LVN/LPN, nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Transportation: Reliable transportation and valid auto liability insurance.
Education:
- Minimum of High school diploma or equivalent
Language skills:
- Ability to read and write English.
- Bilingual Preferred

Physical Requirements:
