**Must reside in the state to qualify for the position** Position Summary: Care Hawaii is a behavioral health care organization seeking a compassionate and dependable Case Manger to support the daily functions of CAREs provision of services. CARE Hawaii, Inc. contracts with multiple stakeholders to provide an array of services. The Case Manager is a member of a multidisciplinary team, responsible for organizing, coordinating, monitoring all contract operations under the direction of the Director.
The primary duty of the Case Manager is to promote consumer recovery into independence by providing an essential range of intensive community-based case management and rehabilitative services for those with high utilization of services and homeless individuals. The Case Manager is a member of the interdisciplinary team that is responsible for assessments, the development and monitoring of the Individualized recovery plan (IRP) and shall act as the communication liaison and advocate internally and externally with respect to the IRP. The Case Manager to consumer ratio is 1:9, the ratio assumes a caseload mix of consumers with varying needs and that most of the population of the caseload is relatively stable and in active stages of recovery; under the direct supervision of the Team Leader/Psychiatrist or APRN-Rx. This member may work in collaboration with other providers and any other members of the consumer’s circle of support.
Position Qualifications:
- Minimum Education: Bachelor’s degree from a nationally accredited college or university in one or more of the following fields:

- Ensure that initial face-to-face intake contact shall be made with each consumer, within twenty-four (24) hours of referral from short term crisis stabilization; within three (3) days of referral when the consumer is in a hospital; and within five (5) days of referral from community agency

- Use alternative outreach and engagement approaches in response to varied consumer needs, cultural backgrounds, life stages, or linguistic needs
