JOB DESCRIPTION

Position: Community Outreach Coordinator

Location: Montebello Administrative Office (Los Angeles County)

Program: Senior Services

Salary: $19.50 - $22.00 Per Hour

FLSA Status: Non-exempt (40 hours per week)

Supervised by: Program Director, Chief Operation Officer, and VP of Programs & Services

General Purpose:

Community Outreach Coordinator will advocate and conduct community outreach for seniors and persons with disabilities and families seeking care management programs and/or public benefits. Will provide information and assistance, referrals and case management to all program participants, walk-ins included, as needed.

Responsibilities:

Provides social service information and assistance to program clients/participants.
Provides program participants with the certification/re-certification of forms for the Cal Fresh Program, Medical/Medicare Program, and Social Security Program.
Provides community outreach to program clients/participants.
Provides referrals, initiates referral form process and schedules appointments for program clients/participants.
Discusses cases with the Program Director for purpose of mandated referrals to Adult Protective Services when required.
Provide interpreter service for program clients/participants.
Maintains an accurate calendar of events, appointments, and schedules.
As directed, translates correspondence, memorandums, forms, documents, and other materials. Composes and edits letters, designs flyers, types and reproduces forms and maintains adequate supplies of forms.
Insures confidentiality of all records.
Must be sensitive to the needs of clients.
May be assigned to attend conferences and workshops.
Answer telephone takes and relays messages.
Assist with community events, and special events.
Performs other duties as assigned.

Qualifications:

High School diploma or equivalent,
Bilingual English and Spanish, preferred,
Strong mathematical skills and ability to accurately calculate,
Must be detail oriented and have a high level of accuracy,
Must be able to multi-task and prioritize,
Ability to work in a fast-paced environment,
Customer service oriented and pleasant phone manners,
Ability to handle sensitive and confidential issues/matters,
Sensitivity and ability to work well with diverse social, ethnic religious and cultural groups and communities,
Must have positive attitude with a desire to work with and promote the welfare of children and their families,
Sincere commitment to provide quality service to the public.

Health Requirements

Must meet State health requirements, including undergoing a physical examination at the time of employment, and must have verification of T.B. clearance at the time of employment (issued within six months prior to employment to be renewed according to Agency Handbook requirements.
