**Position Overview:**
As a Home Health Physical Therapist, you will be responsible for evaluating, developing, and implementing individualized treatment plans for patients in their homes. Your expertise will be essential in assisting patients in restoring their physical function, reducing pain, and maximizing their overall independence. You will work closely with a multidisciplinary team of healthcare professionals, providing invaluable guidance, support, and education to patients and their families.
**Job Responsibilities:**
- Conduct comprehensive assessments of patients' physical conditions and functional abilities in home settings.
- Develop individualized treatment plans and goals based on assessment findings and patient-specific needs.
- Provide evidence-based physical therapy interventions, exercises, and modalities to promote optimal recovery and independence.
- Monitor patient progress and adjust treatment plans as necessary, ensuring timely and effective interventions.
- Educate patients and their families on proper techniques, exercises, and strategies for self-management and injury prevention.
- Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated and holistic patient care.
- Maintain accurate and up-to-date documentation of patient evaluations, treatment plans, progress notes, and outcomes.
- Adhere to professional standards, ethics, and regulations, maintaining confidentiality and respecting patients' rights and dignity.
**Qualifications:**
- Valid and active license as a Physical Therapist in the state of California.
- Minimum of 2 years of experience as a Physical Therapist, preferably in a home health or related setting.
- Strong clinical skills and expertise in assessing and treating a wide range of musculoskeletal and neurological conditions.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
- Self-motivated and capable of working independently, managing a caseload effectively.
- Reliable transportation and willingness to travel to patients' homes throughout the San Francisco area.
- Proficient computer skills and familiarity with electronic health records (EHR) systems.
