About the Role:
MCMC is searching for a Mental Health-Behavioral Health Provider (Integrated Care Therapist) to join the BHI team.
Therapist is responsible for facilitating the integration of behavioral health into the primary care team to help recognize, treat and manage psychosocial/behavioral problems and conditions. The position provides interventions directed toward the prevention, treatment, or management of physical health problems as they correlate with behavioral health concerns.
The therapist will provide direct clinical service to clients including: screenings, assessment, brief intervention, individual/family counseling, and possibly group counseling.
We are looking for someone who is comfortable working: with a wide range of ages and needs, as a member of a multi-disciplinary team, has experience with substance abuse treatment, willing to engaging clients face to face in office and virtually.
Responsibilities Include:
· Provide comprehensive assessment and diagnosis of behavioral health patients.
· Provide effective treatment planning to assist patients in successfully achieving goals.
· Evaluate crisis situations and apply appropriate interventions (we do not provide mobile crisis or in-patient services).
· Participate in meetings that support the integrated health care model to provide comprehensive patient care.
· Assist in the detection of “at risk” patients and develop plans to prevent further deterioration.
· Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education.
· Provide assessment, consultation, and brief intervention for psychological/psychiatric problems, individual/family/couple counseling.
Requirements:
Education/Experience: Must be appropriately licensed or certified and have a master’s degree or Ph.D. in the behavioral/social sciences with a minimum of one year clinical experience.
Certificates and Licenses: Licensed in the State of TN as a Licensed Clinical Social Worker (LCSW), or as a Licensed Professional Counselor (LPC).
This position requires background check and drug screening. Valid TN driver’s license.
