HIV Patient Navigator
JOB QUALIFICATIONS
- Bachelors Degree Preferred
- Extensive knowledge of issues facing people who are HIV positive, hep c positive or who use drugs
- Self-starter
- Harm Reduction outreach or similar work with the target population
- Must have the ability to engage IDUs and their sex partners in informational, non-judgmental discussions about HIV, hepatitis, and related health issues
- Must be able to read and write in English at a functional level
- Must be able to speak with and understand clients and other service providers and use a telephone
- Must be a solutions oriented person
- Must be able to advocate for clients with medical providers, treatment providers, police, and probation officers
- Ability to maintain consistent records
- Good communication and listening skills. Must maintain strong confidentiality and appropriate boundaries
- Must be sensitive to multicultural issues of race, ethnicity, gender, and sexuality.
- Ability to work in a stressful, fast-paced environment as part of a team

Other Skills, Knowledge, and Abilities When appropriate during regular outreach activities must be prepared to explain the general purpose and structure of the program to community members who are not part of the target population (such as drug treatment counselors, attendees at a health fair etc.)

JOB DUTIES:
- Conduct the intake process, interviewing new HIV/AIDS clients and assessing the level of need for services.
- Perform assessments at regular intervals, including health enhancement strategies such as nutrition, exercise or stress reduction techniques.
- Assess HIV client transmission behaviors at regular intervals, providing risk reduction strategies and behavior change support for clients and partners.
- Serve as medical resource person for HIV clients requesting assistance with lab interpretation, nutrition, medication review and HIV/AIDS treatment options.
- Act as a medical referral source for health-related services (ie: dental, nutritional, psychosocial and complementary therapies).
- Advocate for clients with healthcare providers, drug companies, accompanying them to doctor’s appointments as needed.
- Provide liaison between supportive services with related healthcare specialists, social service agencies, and clinic systems.
- Case Manage AIDS waiver clients
EDUCATION:
- Assist physicians with family education as well as referral services.
- Disseminate the most current and accurate information about HIV and its treatments to the population and the community
- Patient Education and Risk Reduction
- Develop or assist with educational programs or workshops that are health prevention related. Presentations to community groups and classrooms.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
Schedule:
- 8 hour shift
- Monday to Friday
