Qualifications:
· Bachelor’s degree in social work.
· At least one year of social work experience in a healthcare setting is preferred.
· Current Social Work license in the state of Texas.
· Excellent written and verbal communication skills.
· Excellent interpersonal and customer service skills.
· Proficient with Microsoft Office Suite or related software.
· Current knowledge of state and federal laws and regulations that apply to the practice of social work in long-term care preferred.
What it takes to be a Social Services Director - Social Worker:
The position of nursing home Social Services Director will coordinate and provide social services for residents. The services will be provided in accordance with current federal, state, and local guidelines and our established mission, values, and policies, to assure that the emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities:
· Works closely with resident, family, and team members in the development of the treatment and discharge plan, based on the psychosocial assessment and accepted social work interventions.
· Responsible for coordinating the following referrals: psychiatric, psychological, vision, dental, hearing podiatry, and following up with the services during the resident’s stay.
· Assess for changes in mood or behavior. Formulate behavioral intervention strategies with residents, families, and staff as needed. Intervene with the resident, family, and staff to work toward a solution for behavioral issues.
· Provide support and resources focused on social/emotional needs, adjustment issues, separation, loss, dying, and bereavement to residents, families, and staff.
· Work collaboratively with a multi-disciplinary team to ensure optimal care for residents.
· Schedule and attend care plan meetings.
· Effective discharge planning to safeguard successful transition back into the community.
· Advocates for and upholds the rights of each individual resident.
· Provides positive customer relations in dealing with residents, families, staff, referral sources, and community partners.
· Maintains accurate and timely records including clinical, MDS-social service, and progress notes according to department policy.
· Completes required forms and documents in accordance with company policy and state and/or federal regulations.
· Ensures compliance with applicable legal, regulatory, and accreditation guidelines and standards.
Physical Requirements:
- Prolonged periods of sitting at a desk.
- Must be able to lift up to 15 pounds at times.
**Texas temporary Social Work license will be accepted. Hiring is contingent upon pursuing a full license.**
