Hospice at Home of Texas is a new company looking for a Speech Therapist that is PRN
The Hospice Speech Therapist provides skilled therapy interventions and treatment to patients in the home setting. Monitor treatment plans and progress towards goals according to the physicians’ order. Provides education to patients and caregivers
Responsibilities:
- Provide skilled speech therapy and/or audiology in accordance with the physicians’ order.
- Complete assessment of the patients’ level of functioning by applying appropriate tests for speech, hearing, swallowing and language disorders.
- Initiate diagnostic, preventative and rehabilitative procedures as appropriate to the patients’ goals.
- Evaluate progress and communicate with the interdisciplinary team.
- Educate patient and caregivers in speech therapy modalities.
- Complete education and participate in education in-services.
- Promote coordination of care through interdisciplinary case conferences and communication with interdisciplinary team.
- Documents care in medical record. Documents at point of care.
- Participate in the Quality Assurance Improvement Program.
Qualifications:
- Successful completion and graduate of a speech therapy program at a school of speech therapy approved by the American Speech Therapy and Hearing Association.
- Licensed as a speech therapist in the state of practice.
- Minimum of one year of experience as a speech therapist preferred.
- Maintain current CPR.

Salary: $35.00 - $60.00 per hour
