KEY JOB RESPONSIBILITIES:
· Educate members about Alpha Comfort Care services and their benefits
· Guide referrals through the initial enrollment procedure.
· Entering client demographics and authorization into HHAExchange and to GA we
· Verify potential Consumer Medicaid Eligibility.
· Conduct follow-ups with all referrals, including those from the media management team.
· Obtaining the Consumer's doctor's orders and signed Plan of Cares
. Working with Care Coordinators
· Manage the referral-to-active home care case and the conversion process.
· Address inquiries, both via phone and email, gathering necessary medical, demographic, and insurance data.
· Ensure accurate and prompt record-keeping regarding admissions, discharges, and other caseload changes.
· Act as a liaison with family members, referral sources, and the community.
```Requirements:```
- Previous experience EDWP-CCSP
- Familiarity with medical records systems and patient service procedures and GAMIS
- Proficient in using phone systems and scheduling software
- Good Computer skills
- Strong attention to detail and organizational skills
- Excellent communication and interpersonal skills
- Ability to handle sensitive information with confidentiality and professionalism
