POSITION SUMMARY
The Referral Coordinator will be responsible for coordinating and tracking referrals from specialty pharmacies to nursing staff. The referral coordinator will work closely with the nurse recruiters in trying to find coverage for new referrals. This is a work from home position, but requires excellent social skills, communication skills, and being computer literate. Communication is required for all aspects of this job. The coordinator will have to speak with the pharmacies, the nursing staff, and patients on a daily basis. The coordinator will take on the responsibly and must ensure that all requirements have been met before sending a nurse to a home. Must promote patient satisfaction and positive outcomes of care.
Position Requirements
1.) Must have a valid LVN and meet the requirements of your state.
2.) Knowledge of medical terminology
3.) Familiarity with computer skills. Microsoft Outlook experience preferred.
4.) Demonstrates strong interpersonal skills and excellent communication skills both written and verbal, as well as, exhibiting strong organizational skills and work ethic.
5.) Demonstrates a professional demeanor when responding to patients/nursing staff.
6.) Must be available for telephone calls or respond back to missed calls within a short time
period. Ability to Video chat and join meeting via Microsoft teams.
Essential Functions and Responsibilities
1.) Coordinate referrals from specialty pharmacies and prioritizing the referrals according to
defined levels of urgency, scheduling patient appointments as necessary and communicating
referral information nursing staff.
2.) Maintain nursing staff for each patient, coordinate with other nurses in the surrounding area if a nurse is unable to see their patient.
3.) Provide areas that are needing coverage to nurse recruiters.
4.) Establish and maintain a good rapport with pharmacy contacts. Respond to all calls and
emails within a short amount of time.
5.) Ability to take pharmacy and doctors orders.
6.) Ability to keep everyone in the loop and pass on information when needed.
7.) HIPPA Compliant. Demonstrates protecting the confidentiality of patients information.
8.) Performs all duties in compliance with all federal and state regulations.
9.) Serve as a positive, enthusiastic, professional, and proactive member of the Health Care
Home team.
10.) Other duties as assigned
