Job Summary:
We are seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for providing support and guidance to individuals in need, coordinating services, and advocating for clients.
Duties:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide counseling and support services to clients dealing with social, behavioral health, addiction, or child welfare issues
- Utilize motivational interviewing techniques to empower clients towards positive change
- Offer crisis intervention and management when necessary
- Supervise and monitor client progress and compliance with treatment plans
- Collaborate with other professionals and agencies to ensure comprehensive care for clients
- Assist clients in accessing resources such as Section 8 housing or other social services
Qualifications:
- Bachelor's degree in Social Work, Psychology, Counseling, or related field (Master's degree preferred)
- Proven experience in social work, behavioral health, addiction counseling, or related fields
- Strong mentoring and supervising skills
- Knowledge of crisis intervention strategies
- Familiarity with child welfare services
- Ability to work effectively in a fast-paced environment
