Oregon Continental Home Health Care is a small home health company with a rapidly expanding patient base whose focus is on the underserved in the community. We are seeking a skilled and flexible OT to provide home care services to various adult populations in Multnomah county. The current area need is N/NE Portland into downtown Portland. Full-time employees are guaranteed 40 hours with drive-time included. Typical caseload is 25-30 points a week (depending on the amount of visits/evals/OASIS) and OTs will schedule their own patients for increased flexibility within their work week. OTs will collaborate with other OT/COTAs, PT/PTAs, and RN/LPNs within the company to problem-solve and provide optimal care to patients.
There is a $1,000 sign-on bonus of which $250.00 payable after 60 days of full-time employment, $250.00 payable after 120 days of full-time employment, and 500.00 payable after 180 days of full-time employment. Rate of pay is based on experience and starts at $60-$62 per hour depending on experience. We offer 1-week vacation after 1 year of full-time employment with paid sick time accruing after 90 days of work. We also offer benefits such as life, health, vision, and dental insurance. $50 gas bonus given weekly when a full schedule is seen as well as $500 yearly bonus to use for CEUs, scrubs, etc. Prefer Home Health Care experience but will train the right candidate.
The Occupational therapist will:
- Conduct patient assessments and evaluations to determine their physical condition, limitations, medical histories, and personal goals
- Communicate with physicians and other health care providers to confirm plans of care
- Advise family members and other supportive individuals about the level of care the patient needs
- Provide occupational therapy guidance, support, and education during patient sessions
- Teach patients how to interact with adaptive technology
- Show patients how to use assistive devices
- Assist patients with purchasing products to help them achieve their goals
- Guide patients through graded activities
- Generate reports for patients and their physicians and families
- Visit patients' homes to help install or use assistive devices
