The RN/LPN charge nurse position is responsible for performing a variety of duties to provide quality nursing care to residents and coordinate care for residents; participate in establishing specific goals; for ensuring compliance with all operating policies and procedures and federal and state regulations; for communicating with' and supervising staff; for providing daily reports and other reports; and for performing all the duties of a certified medicine technician when required.
Essential Functions
Perform various duties to provide quality nursing care to residents and maintain or attain the highest practical level of functioning and to coordinate nursing care for residents illustrated by the following:
- Assesses residents and analyzes obtained information to develop appropriate measures to prevent decline; behavioral changes; records signs of change in condition; notifies appropriate staff per The Bluffs protocol to evaluate weight loss; decline in ambulation, development of skin break down; notifies appropriate staff for possible need of restorative programs; informs attending physician or Medical Director of changes when appropriate.
- Responds to emergencies and resident change in condition; assesses the condition of residents and decides on type of medical attention required; calls appropriate ambulance service if needed; notifies families when resident is transferring to or being admitted to hospital; notifies the physician and appropriate care team members when a change in condition occurs.
- Administers medications and treatments to residents; places orders for medication, treatment and equipment with the pharmacy.
- Maintains resident electronic health records; recording changes and keeping documentation updated. I. Auditing charts as assigned. ii. Updating care plans1.
- Immediately contacts appropriate leadership when residents report abuse or neglect.
- Completes employee incident forms.
- Makes rounds to residents daily and as needed.

Supervisory Responsibility
This position supervises nursing assistants, certified nursing assistants and certified medicine technicians.
Work Environment
This position operates in a nursing home environment. This position is routinely exposed to common health care conditions, noises, chemicals etc. This position routinely uses standard office equipment such as computers, phones, photocopiers and fax machines.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee is occasionally required to sit; climb or balance; and stoop, kneel, crouch or crawl. The employee must frequently lift or move objects weighing up to 10 pounds and occasionally lift or move objects weighing up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
Position Type/Expected Hours of Work
This is a full time position (1.0 FTE) in a healthcare environment which is open 24 hours a day 365 days per year. This position is regularly required to work long hours and frequent weekend work. Holiday and work during inclement weather will be required.

RN/LPN Required Education and Experience * Graduation from an accredited school of nursing.
- Registered Nurse or Licensed Practical Nurse with current state licensure.
- Current CPR certification or the ability to obtain it within 30 days.
Preferred Education and Experience
1. Long-term care experience.
