Dallas Nephrology Associates (DNA) is a large independent single specialty practice with numerous offices located throughout the Dallas-Fort Worth metroplex. DNA has an immediate opening for an experienced nephrology Licensed Medical Social Worker to work with post kidney transplant recipients and their families. Services to this population will include, but not be limited to, ongoing assessment of a patient's strengths and needs to maintain a kidney transplant. The social worker will help patients in problem-solving as it relates to many areas including return to work, access to medication, transplant pharmacy networks, changes in family role, mental health assessment, crisis management and advance care planning. This position is an integral part of the post transplant clinic and will assist the transplant team in supporting the patient to find support and concrete resources as required to maintain the transplant. The social worker undertaking this role will need to be able to triage psychosocial problems daily and multi-task as well as establish and maintain working relationships with multiple community organizations and health care networks.
Qualifications include:
- 5+ years experience as a Licensed Medical Social Worker
- Current valid licensed Social Worker
- Dialysis and/or kidney transplant population experience is preferable
- Ability to triage psychosocial problems daily

Pay: $70,000.00 - $74,000.00 per year
