Our Medical Social Workers are vital to our patient experience for those utilizing the hospice benefit. As a Hospice Medical Social Worker, you will be leading our patients and their families through the navigation of end-of-life care. You will work closely with our interdisciplinary team to prioritize our patients’ psychosocial well-being.
Job Description
- Supporting clients with resources
- Providing education to patients and caregivers about coping skills, hospice philosophy, and non-pharmacological symptom management strategies
- Mediating conflicts within families, between clients and the interdisciplinary team, and between service organizations
- Advocating on behalf of the patient and family
- Identifying and linking patients with resources
- Documenting social work activities
You will spend much of your time in the home with our patients and their families facilitating advance care planning and lifespan planning. Hospice is a benefit that reaches far beyond our patients, and we consider it a privilege to offer these resources to people in their hours of need.

Qualifications:
- Bachelor's or Master's degree in social work from a school accredited by the Council on Social Work Education
- Licensed by Oklahoma State Board of Licensed Social Workers
- A minimum of 2 years’ experience in home health or hospice preferred
- Understands Hospice philosophy and issues of death/dying
- Experience in health care required
- Possesses great computer skills
