Nurse Practitioner
JOB SUMMARY
The Nurse Practitioner will be required to work primarily in non-clinical settings and provide medical care to all levels of patients. (Southside/Westside Chicago Location)
Perform comprehensive medical assessments, order appropriate tests/procedures for diagnostic purposes, formulate treatment plans, obtain specialists' consultations as needed and do appropriate documentation as required.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide general medical care and care coordination to various and/or specific patient levels.
Perform comprehensive evaluations including history and physical exams for gaps in care and preventative assessments.
Establish and document reasonable medical diagnoses.
Create and implements a medical plan of care.
Perform face to face in person visits in a variety of settings including home, skilled nusring facilities, and public locations.
REQUIRED EDUCATION:
Nurse practitioners must earn a master's degree from an accredited program.

● Ability to interpret and adhere to policies & procedures in accordance with company practices.
● Analyze, interpret and communicate complex issues and recommendations.

● Use of basic office equipment (e.g., photocopier, voice mail, ‘fax’ machine, calculator, multiple line telephones, computers etc.).
