About the Role:
- You will work in a radically different model of healthcare
- Serve as the initial Cityblock connection to a prospective member
- Complete the intake and consent process for eligible members, and handoff to the Community Health Partner upon full enrollment
- Locate and meet with eligible members in their homes, neighborhoods, and the Cityblock health hub; nearly all of your work is done in the field, seeing members for scheduled and unscheduled visits. This includes traveling to their homes, accessing their space, sitting with them to explain Cityblock services and consenting eligible members to the program.
- Reviewing pre-populated demographic information and documenting accurate information and emergency contacts within Commons.
- Work collaboratively with your Outreach team to increase the number of monthly Cityblock member enrollments
- Leverage available data from our EHR platform, Commons to learn more about potential members and inform your outreach strategy
- Engage and foster trust quickly with members, acting as the face of Cityblock and the key point-of-contact to assist members prior to full enrollment
- Meet with members in their homes, neighborhoods, and the Cityblock health hub; nearly all of your work is done in the field
- Work daily within Commons to collect data, organize information, track tasks, and communicate with staff, patients, and family members
- Provide feedback to the product development team, and, over time, becoming part of a super-user group to assist in onboarding and supporting others
- Work a full-time 40 hour week, Monday-Friday 9am to 5pm ET
Requirements for the Role:
- You have 1+ years of customer service or call center experience in one of the following industries: retail, hospitality, healthcare, social services,
- You have a metrics-driven approach to your work and are willing to do whatever it takes to meet quotas for the Outreach team
- You are comfortable with high-volume outbound calls as well as creative outreach
- You are excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care
