JOB DESCRIPTION – Graduate Practical Nurse (GPN)

Reports to: Registered Nurse
Position Summary:
The purpose of this position is to supervise, provide and coordinate nursing care in compliance with the facility’s policies and procedures, and to assess resident care needs and take appropriate action.

Graduate from an accredited practical nursing program.

teaching, observational, reporting, monitoring, delegating, and leadership skills.
Supervisory Responsibility: None
Essential/Primary Function of the Job:
1. Under the direction of the nurse manager or registered nurse and following specific care plans, performs a broad range of nursing functions including administering of medications and other related protocols and assignments.
2. Maintains various records and reports reflecting care given such as nursing notes, charges, receipts for drugs, and other matters that are a part of unit procedures.
3. When requested, oversees, and supervises the activities of subordinate staff including CNAs. Makes daily assignments and prepares other schedules.
4. As part of the standard routine, observes conditions, physical changes, and other significant events of residents making necessary documentation and reporting observations and recommendations to the supervisor for review and action. Performs rounds with physicians as requested as part of patient assessment and evaluation.
5. Oversees various services performed with residents including the meal service to ensure residents are comfortable and meals are delivered in the prescribed manner.
6. Assists supervision in orientation and training of new staff and other administrative assignments requested.
7. Follows standards of nursing practice, policies and procedures related to medications, documentation, treatments and resident care
8. Keeps Head Nurse/Supervisor informed of resident, family or staff issues
9. Attends required education programs within the facility and assigns unit staff to attend programs
10. Follows infection control and safety policies and procedures
11. Follows BHNRC’s Corporate Compliance Regulations
12. Carries out other duties as assigned
Equipment to be Used:
General office equipment, basic nursing equipment, medication cart, treatment cart and supplies, rehabilitation equipment, syringes, O2 equipment, suction machines, medication and pill accessories, tube feeding equipment, pumps, IV equipment, nebulizers, mechanical lifts, enema equipment, otoscopes, beds, scales, bed and chair alarms, key pads for alarms, lift devices, pulse O2 units, restraints, gait belts, glucose monitors, tracheotomies and supplies, ostomy products, urinary catheters and bags, infection control equipment, fire equipment and alarms, and other equipment that is required for the care of residents in a long term care facility.
Mental and Physical Requirements:
Full range of motion including handling and lifting of residents, manual finger dexterity, eye-hand coordination, standing and sitting for extended periods of time, ability to ascend and descend two flights of stairs, no restrictions in weight lifting, vision and hearing to normal range, ability to communicate orally and in writing, written skills above average, and able to do simple math and analyze complex information.
Working Conditions:
Work is performed on the nursing unit with other staff, residents and families. Some exposure to communicable disease, toxic substances, medical preparations and other conditions of a clinical environment in long term care.
