You will receive a $1,000 sign-on bonus upon successful completion of 6 months of employment!
Key responsibilities include:

Utilize a team-based approach in facilitating Child and Family Teams and meetings

Provide face-to-face contacts with youth and families in the home or community on a monthly basis, in addition to ongoing telephone contact

Provide crisis on-call coverage for PCE as needed (PCE provides 24/7 on-call crisis services for all PCE clients)

Coordinate and facilitate comprehensive assessments of strength and needs of children and families in their community

Serve as a collaborative partner in the implementation and development of ISP’s (Individual Service Plans) for children and their families utilizing the wraparound model of care

Coordinate Care Management across multi-systems
Qualifications and Skills:
-
BA in a related field, such as social work, counseling, psychology, criminal justice or special ed., and a minimum of one year related experience. MA in related field preferred.

Professional experience as a Social Worker, Case Worker, or Case management background in Human Services

Experience with community relations and resource development are preferred.

Bilingual in Spanish, Portuguese and/ or Creole a plus

Prior experience in managing wrap-around services a plus

Must possess excellent writing skills

Must be computer literate

Knowledge of the Essex County community preferred

Must possess a valid driver’s license
